Are you at risk of becoming homeless after leaving hospital?
If you are being discharged from hospital but cannot return home or don’t have a home to go to, you should discuss your situation with the hospital nursing staff and discharge team. The discharge team will discuss the housing options with you and may refer you to the council. The discharge team will ask if you to agree to be referred to the council and if you consent, your information will be shared with our housing team. Upon receipt of your referral, one of our housing officers will contact you to discuss the housing options available to you and will assess whether you are eligible for temporary accommodation.
If you leave hospital and are rough sleeping, we will refer you to our homeless outreach team who will work with you to find accommodation.
If you cannot return home as your property needs adapting
If you have a home already but it needs to be adapted, the hospital will refer you to Oxfordshire County Council who are able to help with adaptations or Disabled Facilities Grant. Before you can be assessed for a DFG your needs must be assessed by an Occupational Therapist. You can request an assessment by calling 0345 050 7666 or email socialandhealthcare@oxfordshire.gov.uk
